Navigating the Patient Scorecard

Overview

This guide outlines how to access and navigate a patient's Scorecard within the Clinware portal. Users can view detailed patient information, perform PDPM analysis, and manage referral decisions efficiently from this central dashboard.

Prerequisites & Permissions

  • User Role Required: [Check: Admissions Coordinator / Clinical Reviewer?]
  • Dependencies: Active Clinware account and access to the Referral Portfolio.
  • Time Estimate: 5 minutes

Step-by-Step Instructions

  1. Navigate to Clinware.com and click Login, or go directly to the secure portal at https://snf.clinware.com/.
  2. Select the Referrals tab from the main menu.
  3. Click on a Patient Name listed in the Referral Portfolio to open their Scorecard.

  4. Click the Key Indicators section to view critical details, such as Allergies Present, Isolation, Clinical Capabilities, and Consolidated Billing Exclusion.
  5. Select the Scorecard tab in the left navigation panel to return to the main view.
  6. Click on a dashboard tile (e.g., PDPM Analysis, Medications, Diagnosis, or Respiratory) to open detailed data views.

  7. If viewing PDPM Analysis: Click the Show Breakdown button (down arrow) under the Conservative and Potential cards to reveal detailed Reimbursement and Cost Calculations.
  8. Click the corresponding flip cards (e.g., Physical Therapy, Nursing, NTA) to view component-specific details.
  9. If viewing Medications: Review the right-hand columns to compare current costs against alternative generic brands.

  10. Check the bottom right corner for the sum of Total Existing Cost vs. Total Generic Cost.
  11. If viewing Diagnosis: Scroll down to view Clinical Capabilities, Denial Reasons, Comorbidities, and Secondary Diagnosis.
  12. Use the Collapse All / Expand All option on the right to manage the view of long lists.

  13. To Export: Click the Print button (drop-down arrow) in the upper right corner to View a Summary, Full Report, or Download to PDF.
  14. To Decision: Click Accept or Deny in the upper right corner to finalize the referral decision directly from the Scorecard.

Validation

  • The patient's Scorecard loads with all data tiles visible.
  • You can successfully download a PDF report or log an Accept/Deny decision for the patient.

Troubleshooting / Common Issues

  • Data Missing: If specific tiles (like PDPM) are empty, ensure the patient data has been fully synced from the referral source.
  • Browser Issues: If the Flip Cards do not rotate, clear your browser cache or ensure JavaScript is enabled.
