Manually Create a New Referral

Overview

This guide explains how to manually create a new patient referral within the Clinware platform. This process allows you to upload necessary patient documentation and establish a new patient record, using either manual entry or Clinware's auto-capture features.

Prerequisites & Permissions

  • User Role Required: [Check: Standard User / Admissions Coordinator?]
  • Dependencies: Digital copies of patient documents (JPEG, DOC, PDF, or PNG).
  • Time Estimate: 5 minutes

Step-by-Step Instructions

  1. Navigate to Clinware.com and click Login, or go directly to the secure portal at https://snf.clinware.com/.
  2. From the main dashboard, ensure you are on the Referrals tab.
  3. Click the blue Create Referral button located on the right side of the screen.

  4. Upload patient documents by dragging and dropping files into the designated area or clicking Choose Documents.
  5. Review the New Patient Name field. You may manually enter a name or allow the software to auto-capture it from the uploaded hospital documents.
  6. Review the Patient ID field. You may manually assign a specific ID or leave it blank to have the system assign one automatically.

  7. Once all files are selected and fields reviewed, click the Upload Documents button in the lower right corner.

Validation

  • A success banner will appear confirming the upload.
  • The new referral will be visible at the top of the Referrals list with a status of "Pending" or "New".

Troubleshooting / Common Issues

  • File Upload Errors: Ensure your documents do not exceed the limit of 10 files per referral.
  • File Size Limits: Each individual file must be 20 MB or smaller.
  • Unsupported File Types: Ensure files are in JPEG, DOC, PDF, or PNG format. If you have a different format, please convert it before uploading.
